That one decision is basically worth 5 small decisions. If you don't side with him there, he sees it as you desiring his family's death, and that is a pretty big strike. It's unfair, but it explains why that one decision has so much weight.
It seems odd, though. He should give you a "i know you think we may be on the same page, but..." type of speech in that scenario.
If you never do anything for him he will refuse to join you no matter what you say and threaten to kick your ass if you mention the word family. If he did this to you, it might be a save game corruption or something.
